Implant passport and instructions for use
The implant passport contains important information for patients and their treating physicians. Healthcare facilities are required to issue an implant passport to patients with patient-specific 3di implants.
To issue a new implant passport (in case of loss, etc.), please contact the clinical facility where the implantation was performed. Only this facility can validate a new implant passport with a stamp and signature and hand over it accordingly.

CT scan parameters and data transfer
Rapid implementation begins with CT images that are as optimal as possible. To enable effective collaboration, we have compiled CT imaging parameters for you. You are welcome to request these from us (link to contact details).
There are several ways you can send us your data. We recommend transferring data via our digital web upload. This ensures that the data reaches us securely, quickly and easily. Detailed information, a quick guide and the link to the web upload platform are available on request.
You are welcome to send us a QR code that will take us to the data portal you use to download the required CT data.
We also accept data on CD/DVD-ROM/USB sticks. Please send these to us by post to the following address:

Online consultation
In complex cases, consultation between the treating or operating physician and the responsible design engineer is a crucial part of the implant planning process. You have the option of consulting with us either by telephone or in an online meeting. Please let us know your preferences when sending us new cases.

What is the implant passport needed for?
The implant passport contains important information for patients and their treating physicians. Healthcare facilities are required to provide patients with patient-specific 3di implants with an implant passport.
It contains details such as the patient’s first and last name, the name of the manufacturer, the name, type and model of the medical device, and a unique product number. The date of implantation and the name of the person responsible and the facility where the implantation was performed must also be entered.
The implant passport enables the product used to be clearly identified without delay. This can be very important in various situations, e. g.:
- Emergency care (in the event of accidents, etc.)
- Travel (proof, e. g. at airport security checks)
- Facilitated communication between doctor and patient (clear documentation, complete medical history)
After the procedure, make sure that you receive an implant passport (patient) or issue one (medical facility) and keep it handy.
What should I do if I lose my implant passport?
To have a new implant passport issued, please always contact the clinical facility where the implantation was performed first. Only this facility can validate a new implant passport with a stamp and signature and issue it accordingly.
